You move to Madrid, you're getting settled, and then one morning you wake up with a fever and realise you have no idea how to see a doctor here. No card, no GP, no clue where to start.

The key that unlocks Spanish public healthcare is the tarjeta sanitaria, the public health card. Without it, the system is almost impossible to reach. With it, you get a doctor, prescriptions, specialists, the lot. Here's how to get your tarjeta sanitaria in Madrid, the documents you need, and where to go, from someone who's set this up for herself and her whole family.

What Is the Tarjeta Sanitaria?

The tarjeta sanitaria individual (TSI) registers you with a GP (médico de cabecera) at your local health centre and plugs you into the Spanish national health system: appointments, specialist referrals, prescriptions, and emergency care. It's the difference between "covered" and "Googling private clinics at 2 a.m."

Who Is Entitled to a Tarjeta Sanitaria?

  • Employees and autónomos contributing to Social Security.
  • EU citizens with a NIE verde.
  • Non-EU citizens with a valid TIE card.
  • All children under 18 registered in Spain. (If you've got kids, this one matters fast.)

What Documents Do I Need?

  • Valid passport or national ID card.
  • Your NIE verde (EU) or TIE card (non-EU).
  • Empadronamiento certificate. This is the one people forget, and it's non-negotiable.
  • Social Security number, if you're working. (Here's our Social Security number guide.)

Where Do I Apply in Madrid?

You apply at your assigned centro de salud, the public health centre tied to your registered address. You don't get to pick a random one, it's based on where you're empadronado. To find yours, check the Comunidad de Madrid health portal at comunidad.madrid.

How to Get Your Tarjeta Sanitaria, Step by Step

  1. Make sure your empadronamiento is up to date. Everything hangs off this.
  2. Find your assigned centro de salud for your address.
  3. Go in person with all your documents (originals and copies).
  4. Ask at reception for the tarjeta sanitaria registration form and hand in your paperwork.

You'll usually be assigned a GP on the spot, with the physical card arriving later. Keep the resguardo they give you, it works in the meantime.

Quick Questions People Ask

Can I see a doctor before the card arrives? Usually yes, once you're registered, using the temporary proof from your centro de salud.

Do I need a job to qualify? Not necessarily. Children and residents in various situations can be covered. The clearest route is through Social Security if you're working.

What if I don't speak Spanish at the centro de salud? Reception is usually in Spanish only. This is exactly the kind of step where having someone make the call or come with you saves a lot of stress.

The Real Question

The tarjeta sanitaria itself is free and not hard to get. Doing it in the right order (empadronamiento first, correct centro de salud, the form nobody mentions) without taking three trips and a sick day, that's the hard part. And nobody wants to decode the health system while actually sick.

That's what we handle at VA in Madrid. I've set this up for myself and my family, so we make sure your empadronamiento is ready, find your assigned centro de salud, prepare your documents, and can call ahead or come with you in Spanish so you go once. Message us on WhatsApp and we'll sort it with you.

Health shouldn't be the scary part of moving here. Let's get you a doctor.
